Front left corner doesn't have 6 ft of ice water shield, which caused leak in my master bedroom, old shingles left above front door,thin plywood installed over existing plywood, chimneydetail poor,middle capping at front not sealed at top. Ridge vent improperly secured,wood not cut,exposed nails Paid $4650.

We have reviewed your complaint . As for the 3/8 wood installed over top of the existing deck on the backside of the roof , this is standard because we cannot install half inch on top of half inch . The deck would be too thick . In total you have a full deck of one inch at the moment which is appropriate.
if you really believe that we did not put any ice and water shield on the roof over the master bedroom - I would be more then happy to open up that section of the roof and install an even better quality membrane underneath the shingles since that is a heavy ice dam area .
as far as the other details , it can be easily repaired . We would be more then happy to assist you with these minor details . Thank you
