Dr. ********************** ********************** is an Ametrias in network doctor and as such is required to provide services at a reduced rate. I had 5 claims (attached) and two were properly processed by Dr. *********************** one was processed after ****************** filed a complaint with Dr. *********************** One claim was filed by Dr. ********************** without the required x-rays or charting necessary to process the claim and the claim was denied because of this. Another claim was not filed at all (see attached claims). It appears Dr. ********************** is attempting to avoid having his fess reduced by his contract with ****************** by not submitting the claims or submitting incomplete claims so they cannot be processed by ******************. It also appears he is attempting to run out the clock by avoiding submitting the claims within 90 days or responding to the denied claim (with the proper x-rays and charts) with in 90 days. I have requested a copy of my bill, medical records and x-rays a number of times and Dr. ********************** has refused to provide them. I have attempted to pay my bill but Dr. ********************** will not provide me with a bill. It appears he is waiting for the claim time limits to expire so he can charge full fees as opposed to his contracted fees. I have attached 9 pages of documents supporting my claims. Dr. ********************** should submit the required claims or waive his fees.
