      In March 2023, I contacted Mindpath (Redondo) to arrange couples counseling sessions w/ *****************************. I confirmed that I would be the patient & that billing would occur through my insurance but that my wife would be attending sessions, as well. We used my wife's email address to receive the appointment reminders. In March 2024, we received bills to our home in my wife's name threatening to send her to collections for overdue balances ($465.47). I contacted Mindpath to alert them that there was an error & that I should be listed as the patient, not my wife, & that billing should have occurred through my insurance (I paid Mindpath my $25 copay). Mindpath refused to correct the error, noted that the provider (*************************) had falsely identified my wife as the patient, & refused to allow me to speak to a supervisor to rectify the situation. My wife never completed any new patient paperwork, did not receive a copy of Privacy Practices, Grievance Policy, or Financial Responsibility Form, nor did she sign a Consent for Treatment or Consent for Telehealth. Further, she never signed any documentation or provided any information aside from her ************ address. No release of information was filled out, yet the office openly spoke to me about session dates/billing (HIPAA violation). I have called the office repeatedly seeking information or guidance on how to correct the error they made but was dismissed & told "there was nothing that could be done." Additionally, communication from Mindpath staff was minimal & delayed. I had to make multiple attempts to reach someone as they did not return requests for call backs. ********************************************************** refusal to correct their falsified medical documents, in addition to their lack of compliance with adequate & correct documentation & release of information is irresponsible, unprofessional, & unethical. I am hopeful that the BBB will be able to ensure medical documents are adjusted to allow for correct billing.

      On August 14th, I had an appt. with my psychiatrist (who I've had since MindPath Health/ Community Psychwas called ************ health, in ***********). Yesterday, 8/17, I called the office to schedule my next appt. They told me my balance was high so they couldn't schedule me. I had reached out to them previously about the price increase, with no response or resolution (why was it suddenly $183 for a 10-min telehealth appointment just to refill my meds, when my dr. is in network?) I distinctly remember them calling in Jan to get my insurance info, and yet, when. I saw my bill in February, I logged into my account and found that they never updated my insurance info. I updated it myself and contacted them again to check the cost... Anyway, since I can't afford to lose access to care in the meantime, I just want ahead and paid $200 of my ~$400 balance on 8/17. BUT THEN, this morning, I noticed they also charged me on (8/15) without my knowledge or consent. I googled to see if they were allowed to do that, and it's unclear to me. But then I searched my inbox and sure enough, I never received notice from them, nor did I receive a receipt. On 8/15, mindpath charged me $182.93 without informing me whatsoever- before or after! (If this was for monday's appt, that's ridiculous. That's the same amount they tried to charge me BEFORE I provided them with my insurance info a SECOND TIME, months ago! ). And, when I login to my account, there's no record of the charge on 8/15. Like it never happened, This place has been a mess ever since mind path took over. I just can't leave my Dr. He's amazing

      On 4/4/2023 I received a letter from Mindpath Health saying that they would no longer accept me as a patient due to that my doctor is not approved in the ******** network. I have attached a recent snapshot showing that my doctor is currently showing as an approved provider in the ******** network. Additionally, my doctor is showing as an approved provider under my Blue Shield network. Furthermore, now that I am on ********, Mindpath Health will not accept a cash (self-pay) payment from me. I have also attached the denial letter that I received from Mindpath and my reply.
