                  We sincerely apologize that ************** felt that her initial appointment didnt go as she wouldve preferred. After speaking with ************ he stated he did complete a full evaluation for the patient. There was a point were they disagreed in regards to the date on where her disability forms to start.Patient wanted the provider to approve and back date to a previous date in which ************ was not seeing the patient for he didnt feel comfortable on doing so. This got the patient very upset and chose to no longer see *************We can get the patient scheduled for another provider but unfortunately our providers will not be able to backdate the forms to a date prior when the patient became there patient.
      On behalf of CPS we do apologize for any inconvenience this could have caused.

      I had an appointment with a CPS provider, and there was no way to cancel the appointment. I called and left several voicemails. No response. I even sent a personal email to my provider. No response. There is no way to cancel appointments via the website or really any way that I could find. Now, I am being billed $150 for a missed appointment. I even called to speak to someone at corporate to tell them about this harrassing billing/cancellation policy. The person on the other end of the line said that the only way for the bill to extinguished is for the individual provider, who I can't reach, to claim it was an error on his side.

      We sincerely apologize that there was not response in regards to voicemails that were left. On our end at the moment can't go back to confirm if the voicemails were left or not. But we have waived the charges on the account and its a zero balance. Once again we do apologize for this error.

      I made an appt ********************* at CPS. We went over my medical history & which meds I had tried. In the form I filled out & during our conversation, I stressed that I was severely allergic to Wellbutrin. Dr prescribed me ************** called to get a different med. I was calling 4x a wk for 2 wks. Each time I was only sent to an answering service to leave a msg. No one ever called me back.I told one of the reps I needed to talk to a doc to get the straightened out. *******, ************ assistant, called me back. She was so rude & unprofessional. When I told her it had taken me 2 weeks to get through to anyone she scoffed & hung up on **** called back & asked to get transferred to a new doc & said I wouldnt be paying for the appt with ********** since proper services weren't rendered. She gave me some info for another doctor at CPS & when I went to ask another question *********************** up on me AGAIN. I had to find another psychiatrist on my own & pay for that appt. I got bills in the mail from CPS, so I called to dispute. I told the billing rep my whole story & then asked to have the bill waived. She said shed take her notes to her ************** would call me back. No one ever called back until almost 2 weeks later but as soon as I answered they hung up. I called back & the woman I spoke to looked at my account & said they called to say **** was being sent to collections.When I explained my story AGAIN & that I had been waiting for the billing manager to call me back, she said there was no record of that. Fast forward to the end where they waived PART of bill but told me I HAD to pay the other part or it would go to collections. So, I did because I want it to be over. I spent so much ************* trying to get a hold of my doctor/her ************** **** to get things cleared up it was exhausting.For a company who is supposed to provide mental health services to individuals struggling, they failed me. I should get a refund & the ****************** should be reprimanded

                  Regarding Case # ********
                  Patient was seen for a full new patient evaluation on 11/2/2022. At the initial appointment the patient was prescribed Atomoxetine 40 mg capsule.  On 11/16/22 the patient spoke with Dr.********; letting her know that she went to the pharmacy. But that Atomoxetine was too expensive for her to purchase, she requested the provider prescribe a cheaper medication.  ********** then prescribed Clonidine 0.1mg instead. The patient was not prescribed Wellbutrin like she stated.
      We sincerely apologize how our staff ******* whom previously was working with us conducted herself. This staff is no longer with our practice but we have made sure to talk all staff so that we can ensure this doesnt occur once again.
      Unfortunately, since services were fully rendered on 11/2/22 the charges do stand and patient was given a discount. Originally, she was responsible for $560.73 for her deductible but due to the circumstances and the complaint patient made a discount was given to the patient of $227. Which that left the patient with a responsibility of $337.73 which patient made payment on 6/14/23.

      I am rejecting this response because:

      I stated the doctor tried to prescribe me Wellbutrin after I had waited two weeks to get in touch with her. But before she sent it through I notified her I was severely allergic like we had previously discussed. Although Im glad ******* is no longer on staff, that doesnt change the harm she and ********** caused me. Not to mention all the time I wasted trying to get a hold of my doctor, the office, and the billing ***** I dont accept that I must pay for an appt in which the doctor ***** listen to me and an appt I did not get anything out of as I had to go see another doctor who listened to me and prescribed me something that worked which is not what I got from CPS.
