      On 08/30/2022 I received a Past Due Statement for services rendered 01/19/2022 in the amount of $358.00. The statement does not reflect insurance being billed. On 08/31/2022 at approximately 12:45 p.m. I called ************ and spoke to a rep who did not give her name. She asked for my account number but verified NONE of my information (not even my name). She did not give clear information as to why the insurance did not pay or was not billed. I tried to get clear ***************** hung up on me! >> I want that call pulled & that rep to be retrained. >> My insurance needs to be properly billed & if the doctors office needs to resend an auth Radiology of Indiana needs to contact them so this claim is paid.

      Thank you for the opportunity to respond to this complaint.  The consumer underwent a radiological service on 1/19/2022.  *********** was billed to Caresource Indiana HIP and subsequently denied for no authorization.  We generated a first statement to the consumer 7/22/2022, without reply from her.  A second statement generated to ******************** 08/22/2022 (both statements attached).  We received a call from her 08/31/22, during which she states we both failed to bill her insurance and failed to clearly explain why she was billed.  In fact, our representative (****) informed her we billed Caresource and that they denied the claim for no authorization.  The consumer further indicates in her complaint that if ROI needs an authorization, we should contact her doctor's ******* confirming she understood the claim was denied for this reason.
      Please note that at the time of service, the hospital in fact sought an authorization and it was denied.  ******************** made the choice to receive the service anyway, accepting full financial responsibility for the service.  The signed "Beneficiary Agreement to Pay" (attached) demonstrated her full knowledge and acceptance of this liability.
