      On January 5, 2023 I purchased a groupon voucher for Jiva Med Spa for 40 units of Dysport for $229.00. Group-on# **********************.(copy attached). I scheduled my appt for January 24th. I arrived at Jiva Med Spa in Beavercreek on my scheduled date. I was greeted by the receptionist to sign in & sign documents. I was then greeted by Dr ****** *****. He asked me to accompany him to a room to discuss my injections. He asked have I had Botox & Dysport before? I told him that I have had both before but I prefer Dysport because it tends to last about 6 weeks longer for me than Botox. I usually get around 50 units of Dysport for it to be effective to reduce lines & wrinkles in my forehead & crows feet around the eyes.During the discussion he told me that 40 units of Dysport is only equivalent to 11 units of Botox. I had never heard of this but continued to listen to his sales pitch. He said he would give me 11 units of Dysport with 50 more units for an additional $300 to ensure I was satisfied with the results. I agreed. He escorted me back out to the receptionist to pay. He then told me he was going to draw up my injections & let the nurse know that i was ready. I was then greeted by the nurse who I want to say her name was ****** or something close to that. She then proceeded to tell me that the doctor has never drawn up any syringes for her which she found odd but was pleased. She completed those injections. Then she stated that I need to give you a few more units close to your hairline. I watched her as she opened the small refrigerator in the treatment room. I watched her draw up more dysport/botox into that syringe. She then injected that into my forehead in three different areas across the hairline. She said i should see results in a few days usually about 4. On day 5, I felt the area that she added work but none of the other areas that was drawn up by the doctor. I contacted them & was told no refund. I went somewhere else & received perfect results.

      ***** ******** was in our office on Jan 24, 2023 with a groupon for 40 units of Dysport. She was seen by Dr. ***** for the consultation, we described the difference between botox and dysport to all our patients. This is pulled straight off the medical website from google:  the conversion from Dysport to Botox is usually 2.5 or 3:1. Therefore, if you usually get 40 units of Botox, then you would require between 100 to 120 units of Dysport. On average,a Dysport injection will last for about three to four months, while a Botox treatment can last from four to six
      We always let our patients know and give them the option to upgrade to botox. 
      ***** wanted to continue with the dysport and paid an additional $300 to add 50 units. She was also given at treatment and additional 60 units by our nurse ***** at no additional charge because ***** knew the patient would not be happy with the results in all the areas she wanted treated with the amount of dysport she purchased. The typical patient would require about  220 units of dysport to treat all 3 areas with deep lines. The patient paid for 90 units between the groupon and additional payment, but was given 150 units total. She was treated with Dysport Lot # W18084  Expiration 5/31/23. When she called the office unhappy with the results we offered to inject additional units at the same discounted price but she wanted a completed refund or additional units for free. We do not disburse refunds for treatments already received and we had already given her 60 additional units for free.

      I am rejecting this response because:
      Their response about any such “free 60 units” is not factual. I am not disputing the amount of alleged Dysport that was sold to me initially. I am saying that it was NOT Dysport or Botox of any kind. It was not real product. I went to another place & received 100 units of actual product & it was 100% effective! I was also NOT offered additional units when I complained. I didn’t see any nurse or doctor my day of complaint only the receptionist. I was told the doctor wouldn’t be in until Tuesday. I’m 100% sure that the product was not real. The doctor is the only one responsible for the fake product! I again request a full refund!

      I inquired about cool-sculpt at a facial appointment I had, Dr ***** looked me over and said I needed trusculpt ID, not cool sculpt. He was so confident I’d get the results I want, I am healthy, 5’5, 128lbs. I just have a little mommy pouch on my stomach. I paid $3200 for this treatment over a year ago followed by Venus treatments up until a few months ago and have zero results. It’s like a sick joke. I talked to him about it and he had the nerve to say, I don’t know why you did trusculpt, you should have done cool sculpt… then said for his special price of $2500 he will do the coolsculpt… at that point I’d have $5700 into my stomach… i could have just got a mini tummy tuck or something else. It’s all a money hungry game to him. He was sarcastic, and made me feel uncomfortable and silly. He lacked compassion and didn’t want to make it right at all. I’m so dissatisfied with Jiva and Dr *****. I’d never recommend this guy. He doesn’t care about you. He just wants your money. At least he could have offered to try another treatment to help me achieve even the slightest results… they also “misplaced” my before pics. Ridiculous.

      ******** **** was seen in our office on 7/6/21. At this appointment she asked about our different weight loss procedures. Trusculpt ID was suggested for her due to the fact she did not have much fat in the abdomen area to loose, as well as tightening up the abdominal area for her would have been a better option. The patient agreed and signed a service agreement to proceed with the procedure. She paid a discounted price of $3200 for the truesculpt that was a $1600 discount. Plus she was given 4 free venus treatments to follow the procedure. On 7/14/21 she has the trusculpt procedure done. Her next treatments were scheduled once a week for 4 weeks starting 9/7/21,9/14/2021 the patient cancelled the 9/21/21 and rescheduled to 9/28/2021, then her last one was 10/5/2021. At this appointment she was not happy with the results of her treatment and was given another 4 venus treatments for free. She was explained the importance of the keeping her appointments as scheduled. She was scheduled for 12/21/21, then 1/18/22 for her second appointment she cancelled due to covid. She did not call back to reschedule this appointment until 5/3/22, but cancelled this appointment as well. She rescheduled for 5/17/2022, had an appointment on 6/21/22 and 8/16/22 completing her 4 extra treatments. On 10/11/2022 she was scheduled to come in as a follow up with but the provider was out sick. She did follow up with Dr ***** and was offered to try a different procedure at a discounted price. However the patient did not wish to continue treatment. We have done all we can for this patient. She has received $9600.00 worth of treatments for $3200. We are no longer able to offer future free services for her and can not refund for procedures performed.
