I went to Concentra on **** ******* blvd in **************** for a work injury, right ankle sprain. Once cleared, I was seeking new employment. My new employer told me I needed my release paperwork to specify that my clearance included driving commercial vehicles, which is my trade and career. I went to get this addendum on my records and was told this was not allowed and that I had to have an entire new appointment to get the paperwork stating this. When I showed for the appointment, which was paid out of pocket, I was told it was the wrong type of appointment and I had to do a new physical. I went along with it, trusting it would get me the paperwork I needed. At the end, the PA gave me a new medical card and when I asked for a note specifying commercial driveling I was informed that it would be illegal for her to do this and that my med card should be enough, even after I was told and told her that it was not. Per the new employers insurance, I needed paperwork with the clarification that she was refusing to give to me while her and the front desk staff were all condescending and treating me like a child instead of an adult who is trying to get a job. I am currently unable to find employment in my field without this note and she continues to refuse to provide any further assistance.Initial Complaint

I went to a concentra urgent care to do a drug screening for my employer. The concentra is on *********** in ****************. There was a black guy there who i went to school with that never liked me so he basically was throwing my paperwork at me and constantly getting smart with me. Then he had a white lady to come remove me from the room and make me leave without talking my drug test or my physical and they banned me from all concentra locations where i cant go to any in ******** anymore but i have a job offer and my job only sends people to concentra but they wont let me come back there. On the day of July 24, 2024 on Wednesday at 11:05 AM I went to Concentra on 10th in Shadeland for a drug screen for velvet ice cream I gave the information to the receptionist for velvet ice cream dress green after they call my name went back to the back and did the screen a younger guy who didnt ask me if the information was correct or anything he sent my personal information to ************************ instead of velvet ice cream. In March of 2024 I was met with poor attitudes and incompetence from staff at Concentra in ******, **. After I asked the *** ******* to provide her managers number, I was then refused any additional blood draws to obtain my labs when 2 minutes prior she told me "They would try one more time" this set the stage for how she proceeded to retaliate against me. She then made false accusations to my company and a 3rd party company with the following accusations:Verbally abusing staff Forcibly grabbing needles from an MA Stating, MAs should not draw blood and RNs are betterThese things did not happen, she then had *********************** one of your Vice Presidents of Medical Operations at Concentra in ******** place a ban on me from ever using a Concentra in the ************* again. This is a rather strong action to take off of false information and false accusations.I attempted to contact multiple levels of HR and Board members to express my concern in which I was told it was "investigated." Essentially, it's just this clinics word against mine in which they have been allowed to fabricate my interaction and deflect from any truth of how poor their staffs attitude was that was reflected in their communication with me. I have let months go by and yet I am still having a hard time moving forward and past this as I absolutely cannot stand false accusations and lies against me. I was very much dismissed. I get it, "it's me vs. a corporate company" but this was wrong and the fact that I attempted to bring a concern to this centers manager, and it has now turned into retaliation and me defending myself from blatant lies and being banned from an entire corporation with locations all across the ** isn't something I can "just get over." I've been struggling with recurrent UTIs and came here this afternoon after testing positive on a couple different OTC UTI tests *********** with sharp pain on my side and the usual UTI symptoms.The lobby was empty when I arrived, and it took 30 minutes to be taken back into the room, where I waited another 45 minutes alone. Then, a male doctor came in and told me that UTIs are a common "women's issue" and that "most women" deal with it by drinking cranberry juice and getting "that AZO stuff," both of which I had already done. When I asked if he could look at the sharp pain on my side, he shrugged and said it happens to women and that I should just take ibuprofen, as I would not be getting antibiotics from him for my UTI. Then he made a comment about how my jewelry looked cheap before ushering me out.I feel scammed, as this facility advertises itself as urgent care, and I now have to wait another day to go to an actual urgent care facility because they took so long that all the urgent cares were closed after the 2 hours I was there.Customer response

Hi, I'd like to follow up on my original complaint to say that I was able to be seen by another provider who ran actual tests for the issues I went to Concentra with and confirmed that not only are they visible infections, but also tested positive for infection. They prescribed me 2 antibiotics.
I have still been reaching out Concentra and have not received a response when asking for a copy of my visit and the test results that they claim to have run.

Hello,I had a preemployment urine screen done at Concentra in ************** on April 9th, 2024. The lady said my temp was out of range and called the employer. After a couple of mins the temperature gauge change to the color. She signed my paper work and I left to go back to *******. A few hours later I received a phone call stating that I had to come back to watch them open my packet to write remarks. I called and asked what was going on and they stated they made a clinical error and I needed to come back but I was almost back to ******* so I couldnt. So a few days went by and they marked in my clearinghouse a refusal when I have my paper work saying I went up there and completed it. Now I lost my current job with fed ex grounds. The stress & overthinking led me wrecking my vehicle. Now Im a homeless veteran of the US Army.Business response

After your visit on April 9, 2024, you were informed via a phone call that you would have to return to the center to complete a subsequent collection as a result of the visit. During this call,you stated that you would return the same day, April 9, 2024. You then called back and stated that you would be returning the next day, April 10, 2024,however, you never returned. In response, we called you several times but you did not answer our calls or respond to any of our messages and as a result, we informed the *** of the events that occurred and that you had not returned. I came here for nursing pre employment physical. I arrived at 8:30am on 5-6-24. I was waiting and registering for 20 minutes when I was told the doctor was pulled and would take 3 hours to get physical. Now I have to return for a physical further disrupting my time.Business response

Thank you for bringing this to our attention and for allowing us an opportunity to respond. When you first visited our center for these services as a walk in, you were informed in the waiting room that it would take approximately three hours to complete all services due to the number of patients that had arrived before you. You decided to complete one service that day and return for the other services another day. You returned another day to complete the rest of the services as a walk in. Unfortunately, walk in volume can be unpredictable and total time in the center can fluctuate based on a variety of factors, including the number of patients that arrive before you.We sincerely apologize for any inconvenience this may have caused. I would like to file a complaint about services at concentra regarding a doctor name ************************* an MD specialist that has failed to proved me the best service possible , ***** did not have the best interest at heart regarding my visit for a injury I was immediately accused of lying and the doctor provided me his personal thoughts regarding my injury and also attacked me in a way I felt uncomfortable Ive even tired walking out for help and the doctor placed his hands on the door telling me I cannot go anywhere I then felt super scared and uncomfortable and most certainly I didn trust his judgment by the way he treated me . Even after asking for help and addressing the situation to other colleagues at the facility I then asked for them to change the doctor I was promise that my next visit it would be with a new doctor and concentrate ignored my request and now that same doctor has cancelled all of my appointments for help and especially utilizing physically therapy after Ive mentioned I have hope in continuing physical therapy that was already booked for me to use then he canceled all treatment and released me to work with no restrictions even though Im still in painBusiness response

Thank you for bringing this to our attention and for allowing us an opportunity to respond. We sincerely apologize that you do not believe that you received the best service possible. Furthermore, ****************** adamantly denies that he accused you of lying, provided you with his personal thoughts,attacked you in a way that you felt uncomfortable, and that he placed his hands on the door and told you that you could not go anywhere. We will be looking into your request to change clinicians at Concentra further. Regarding the claim that all of your appointments (and thus treatments) were canceled and you were released to work with no restrictions, we will provide further information below. When you initially presented to Concentra, your mechanism of injury,symptom complex, and exam that were incongruous. ****************** found the connection between these questionable and informed you of this, however, you were referred to physical therapy and prescribed modified work duty. During a follow-up visit, the same inconsistencies were still present and you were ultimately released from care and returned to full duty.
Again, we sincerely apologize that you do not believe that you received the best service possible. It is possible for you to reopen your case, with the requisite approvals, if you are still in pain and we would be happy to treat you. *** 25th and Jan 26th my husband and I took our daughter to Concentra. On their website and on their phone tree it says "Walk ins are accepted". Yesterday 01/25/2024 we took our daughter in to get her physical for school, during their walk in hours. We were told by staff that they were doing paperwork and they couldn't see her. Today 01/26/2024 at noon we were turned away. The staff said they had different priorities and they turned us away once again. When I called to verify what the website and phone tree says as being accurate, I was given a scripted response. When I asked for the name of the employee that I spoke to she refused to give me her name and said they are not required to do so. When I went into the facility and asked for their names. One employee gave me her name and two others refused and said they are not required to do so. When I called Concentra In ***** where they are headquartered, I made no progress. I would like for Concentra or ************ the Director at Concentra to contact me in regards to why their website and phone tree is inaccurate. Thank you for bringing this to our attention and for allowing us an opportunity to respond. As a result of this complaint, we reached out to the consumer on January 29, **** and left a message, and we were ultimately able to speak to this consumer when we called again on January 31,****. When the consumer visited Concentra on January 25, ****, at approximately 4:15 p.m., unfortunately we only had one provider at that time and said provider had to leave the center. As a result, we requested that this consumer make an appointment or return for a walk-in visit the next day. We sincerely apologize for any inconvenience this may have caused. Furthermore, during the call, the consumer expressed that the complaint was resolved and we provided contact information should the consumer have any future issues.
