I have ******** and am only allowed treatment in **************). This company says that I am responsible for lab work out of state that I never approved or agreeded to cover the costs of. Regardless of who apparently asked the to run labs i never ok'd payment or responsibility. Therefore fraud took place by someone using my identity and agreeing I would pay for someone I never agreed to.Business response

In response to the complaint below I have attached a few documents which clearly details the unfortunate truth to this persons complaint. We are a licensed clinical laboratory and document and retain every piece of data with regards to testing of patients. This patient was referred to an outside collections agency after numerous attempts to collect the reimbursement that was paid directly to the policyholder. Sometimes BCBS will pay the individual directly if the provider is out of network. Typically, the patient will sign this check and mail to our facility. Sometimes that is not the case in which we then send those to collections. This testing was performed in 2016. We referred them to collections in 2019 since no remedy was achieved.
Files attached include:
1. Central Tox billing statement clearly showing the amounts paid directly to patient and where to forward.
2. Collection Agency placement (*******************)
3. Each one of the service dates lab order forms with patient signature on each one.

I am rejecting this response because:
The company stated that the policy holder recieves a check sometimes. Firstly I would like to formally report I never recieved a check. Secondly the as I stated before I never agreed to cover the cost of any lab test and I know without a doubt Central Tox will not be able to able to produce any documentation where I agreed to do this because it doesn't exist.

Attached are the test order forms where the patient clearly authorized those tests. I have circled the location right above the signature line that states payment is authorized directly to Central Tox and patient is financially responsible for payments should insurance be denied,partially paid, or co-payments required.
Sometimes if checks went directly to them and it wasnt cashed they can call insurance carrier to reissue these checks.
My suggestion would be that *************** call his insurance carrier directly and reference the Dates of Service and if he never received the payments directly, to have them re-issue those checks. He can then forward those to us directly to clear this matter up.
