On May 9, 2023 the Virginia Board of Veterinary Medicine found the following violations of the Regulations Governing the Practice of Veterinary Medicine.

1.  No change of Veterinary-in-charge form was filed with the Board.  2. No change of Veterinary-in-charge drug inventory for schedules II - V drugs and no inventory could be produced at the time of inspection.  3. Improperly disposing of schedules II - V drugs.   4.  Expired cytarabine injection in the working stock. 5. The drug distribution records were not in chronological order and missing transaction dates, drug strength, the amount dispensed, administered and wasted, the animal identification and the Veterinarian who dispensed the drug.  6.  Certain drugs require monthly reconciliation for dispensing and no records were available. 7.  Invoices for the drugs did not note the date of receipt and had not been maintained for a period of 3 years from the date of transaction. 8.  At the time of inspection the required biennial inventory of the mentioned drugs had not been completed. 9. Prepackaged drugs were missing required labeling to include the name of the drug, strength, date re-packaged, the lot or control number or the correct expiration date. 10.  The establishment did not have an animal identification system at all time when housing animals. 11. Daily patient records did not include the treating veterinarian, correct date of contact, the drugs administered, including quantity, strength and route of administration.

Dr. Courtney Weigard, Veterinarian in Charge admits to the violations.   For additional information go to:  www.dhp.virginia.gov
