      Transitional Health Services of **********: Where do I start? This place does not deserve zero stars! They deserve negative stars!!! First, the place is a dump, its old and outdated. The staff are rude and do not care whatsoever. The only person that was nice was the lady that works Monday through Friday at the front desk. My father was there from 12/23/2023 until he was taken to hospice 1/25/2024, which should have happened two weeks ago. He has dementia, he is blind, has a brain tumor, and has pneumonia, and an ear infection that the staff let turn into a ruptured ear drum. While he was there these are just a few things that happened:1)He started out in hall 200 and they let him fall out of the bed eight times. The call nurse button (when it worked) was always ignored. To get my dad ice water took one hour and forty-five minutes for someone to come to the room. We went to the nurse station numerous times, and they always said the same thing, well have someone there in just a minute. 2)We would come in at different times to find him lying in the bed with nothing on but a diaper, no cover and he was freezing cold. 3)He could not feed himself because he could not see, and they assured us they would have someone come in and feed him when my mother or I could not be there and that almost never happened. The Director of nursing, ***************************, assured us that his meal tray would not be delivered into his room until someone was there to feed him, and we walked numerous times to find an untouched food tray, hours after feeding time had passed. We had a meeting with ******, and she said she would make sure this stopped but it never did. She was rude to her staff as we observed several times. 4)The hospice had to step in ten days ago and when they were there (four days a week, just for a short time) he was taken care of much better. And today, they got him moved to ****** Hospice House and we were there this evening and let me tell you, it is ***** times better than Transitional Health! All we want is for my dads final days here with us is to be peaceful. Transitional was not giving his medication as scheduled and just did not care. We found meds just lying in his bed side table. We know they could not have given them to him because he can not walk or even get out of bed, unless they let him fall and we wonder how long he laid there in the middle of the night? I left FIVE voice mails for the Director of the center, and he never returned a single call. Red flag there! If you have a loved one who needs assisted living care, PLEASE DO NOT choose Transitional Health Services of **********! You will be so sorry. I am filing a complaint with the ** ******** Ombudsman and the BBB.
