      On 9/16/2022 I visited Access Vision, PLLC (Dr. David *****) to receive my annual eye exam. He performed the exam and I paid for my visit but he refused to sign my wellness exam form for my health insurance carrier. He would not provide a reason to why. I have no history of missing or cancelling my appointments and have received exams from Dr. ***** for years and he has always signed my wellness exam forms. Several other coworkers have complained that he refuses to sign their wellness forms. This form is literally a signature and simply verifies he conducted an eye exam. He performs the exams as always and takes our money for the services but for some reason will not sign the form now w/out an explanation.

      The complaint of not signing an insurance form for a "wellness exam". One year ago Access Vision decided to discontinue "wellness exams". We contacted the insurance company and removed wellness exams from our contract. Every patient with this insurance that calls to schedule an exam we tell them on the phone that Access Vision is not contracted to do wellness exams. When the patient was here for the exam I explained this to him and he was also told before he scheduled the appointment.

      I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this proposed action would not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

      There is no contract with our insurance company regarding “wellness exams”.   If they are unwilling to sign the wellness exam form for their patients, then they need to quit offering wellness exams (general eye exam) to patients in general and stop collecting our copay and payment from our insurance provider when they complete the exams.  Signing a form has not and will not cost them any loss in revenue based on my conversations with Dr. *****.  This is a punitive decision against past and current patients with no explanation to why they decided to quit signing the wellness exam forms.  They conduct the actual exams, but simply won't sign the form.   Only loss in revenue will be losing patients over this because they will find another optometrist to visit.

      I read his complaint and response. He states that his desired settlement is to " finish the job". I gave him an eye exam and completed my job. He was told and others that have this insurance, that I discontinued doing "wellness eye exams". He has the choice to go to any other provider that participates in "wellness exams". He said something about talking to me about loss of revenue and he determined that I would not have a loss of revenue. I told him that I had no comment on revenue and that I simply called the Insurance company and told the Insurance company that I was no longer participating in wellness exams. I can not sign a form for Mr. **** when I am not participating in wellness exams and that would be unfair to sign his form when other patients forms arent signed. I have no obligation with the BBB to resolve any complaints.
