      Dental procedure was scheduled with company as business was covered under insurance provider and would cost nothing out of pocket. Received a bill in mail. Called insurance about bill and was told acting provider at dental office was not covered by them which is a violation of coverage agreement as all providers must be recognized by insurance provider for any business to be covered by U***** ********* (insurance provider). Dental office did not inform us that they were in violation of coverage agreement and that any procedure through them would be an out of pocket cost. There was no attempt with dental office for any informed consent of discrepancies of coverage issues in any point of appointment schedule to actual visit. If there were any issues brought up, the dental procedure would have been scheduled through another business legitimately covered by the provider as per the medical contract obligations with active military service. I have called dental office with complaint on 7 Aug 2023 who said the only doctor covered under our insurance had retired and that they will not help us out with the bill since they had apparently helped cover an earlier visit for our daughter without telling us about any coverage issues.

      My name is Timothy **** I am  the owner of *** Family Dentistry. Linda *** has retired and she is working on closing out her accounts and business. I was never made aware of there being an issue. I did not know there was and issue until the fax was placed on my desk. However, I would like to apologize for this issue, it never should have happened and for that I am sorry. 

      I spoke with the employee who failed to inform me of the issue at hand and they have been given a formal write-up that will be added to their personnel file. Additionally, I will be refunding your payment of $72.16. Ashley should be contacted soon to verify her address so a refund can be sent. I do apologize for any inconvenience.
