      I made an appointment for a consultation re:vericose veins on June 14 2023 at 1:00. The receptionist asked me a number of questions and took copies of my driver's license and insurance card. I asked her about any charges there may be and she said she didn't think so. I asked her to check my insurance. She left the room and shortly after returned with 2 men, all with I pads supposedly checking me insurance. Both said they didn't "think " so and told me the next step was an ultrasound of both legs. I told them I made the appt for a consultation only, but they said if I didn't do the ultrasound they say I wouldn't get as thorough an exam next time. I agreed to it although I felt ganged up on. The ultrasound was very thorough on both legs although I said several times I only have an issue on my right leg. That technician said the doctor would give me the results. I was taken to another room where a man (doctor? He wore no name tag and did not introduce himself) told me (in an accent I could barely understand) that I was a candidate for several procedures, although I couldn't understand any of the names. I asked if he could send me an email with the info so I could do some before I committed to anything. He said he couldn't do that until I booked several dates for the procedures first. I said I did not want to book further apts. He said several times "you don't have to show up" for them but he had to book them before he could give me more info. At that point I told him I was feeling uncomfortable and was leaving. I again told him not to book further appointments. By the time I got to my cars I received a taxi telling me I was booked for 7 more appointments in July and August. I believe this clinic is scamming ******** andtor insurance companies and I wish to file a formal complaint.

      My mother had services provided by this company. Her ******** and secondary insurance has paid the full balance that was owed. She has reached out to both insurance companies and they have confirmed payment to the company. She is still receiving multiple calls from the company a day saying she has a past due balance. I talked to ***** at the ****** location and he confirmed that the insurance company has paid the bill. He said he was submitting a request for research of the payment on 06/08 and would put a note on her file for them to stop calling her. She has continued to receive calls daily from them for a balance of $357.98. That is the exact amount Prosperity sent electronically on 05/10/2023 and the funds were received on 05/12/2023. Please see what can be done to resolve this matter.

      I got a consult at Vein Clinics of America ************ ** location on Jan 20, 2023. I was told the treatment would be $400.I scheduled my spider vein treatment on Feb 22, 2023. The doctor was an hour late but I did finally receive treatment. I paid $410 and there was no mention of any other charges. I was told it could take 3 months to see full results. I see very little, if any, difference in my legs.I received a bill in late April that said I owed another $291.60 which was 90 days past due and before I even got a chance to call anyone, my credit card was billed. I then called to try and get a follow up because I didn't notice any improvement and was told this location was not longer in business.I was offered to go to a location an hour away and they cancelled the day before I was supposed to go in. At this point, I want the $291.60 refunded and I would prefer that another location offer to do a free touch up on some of the most prominent areas that I don't see any change.Today, May 24, I called the main number found online, ************ and tried to tell them about my complaint and a man named ******** said he was going to get someone to help and then I was disconnected. I would appreciate some help in resolving this matter. Many thanks!*************************

      USA Vein Clinics (*** VC) used to be Vein Clinics of America (VCA). When I arrived for my appt on 4/26/23 the nurse took me to the back and asked why I was there. I told her I had some scarring on my legs from the last ablation 8 months ago. I've never scarred before in the past 7 years of treatments. The nurse proceeded to ask me questions like "Who was your doctor?" ***** procedure did you have?" "Did you have injections?" "When was your last ultrasound?" I told her to please check my chart. That's when I was told VCA had gone bankrupt, was taken over by *** VC, and all VCA patient records had disappeared! I was then asked if I'd like to start over with *** VC and take new ultrasounds so that Doctor **** could treat me. I said yes. After a 2-hour process the nurse said all I seemed to need are some injections for spider veins. Then I met with *****************************. I was still in my paper shorts they provided. ************ came in and left the door wide open. He questioned me: ***** medication did they inject you with?" ***** are the names of the ablations that you had?" ***** were the dates of your procedures?" I had no specific answers. He told me to stand and turn around, I was on display so I reached to close the door at that point (he never thought to do that). He repeated multiple times that he had no records and this was no longer VCA. I asked "Where do we go from here?" He said "Nowhere". I asked about the injections needed and today's ultrasound. He said he could do nothing without knowing the meds injected and the types of ablations I had. I said "I've been here for 2 hours and you can't do anything?" He said "I've been here all day!" I complained that I'd be billed for this visit leading nowhere. He said "This is just a consultation". The nurse said "No, she had a full ultrasound so she'll be billed." I called the following day and requested my records, gave my fax # and was told it could take up to 30 days because a different department handles records.

      I was a patient of Vein Clinics of America, which was purchased by USA Vein Clinics in the middle of 2022. At the time of purchase, I had a $967 credit on my account which USA Vein Clinics has confirmed. I was planning on using the credit to pay for future procedures. After the purchase, they told me that location was closed. However, when going to the prior location, they were open and seeing patients when I went to the physical location. After that, over a period of months where I attempted to schedule the procedures approximately a dozen times, I was informed that that location and/or provider did not perform that procedure. After attempting to reschedule at multiple locations with varying providers, they would either indicate on the phone a location did not provide it or they would call back later to cancel that appointment indicating they did not provide it and reschedule. This happened multiple times. Eventually, I concluded I was not going to get the desired service with their company and requested the $967 be refunded. I have called multiple times since September 2022 to request the refund. Initially, no one had information on the credit on my account. Eventually I was put into contact with their billing department who acknowledged the credit and indicated it would be paid out the last week of December of 2022. Subsequently, they indicated the agent who said December was incorrect and said they did not know when I would be provided my money but that another department was handling the refunds manually and that it was very slow. No ETA would be provided. This has continued through present day, April 2023. I've been calling monthly at this point with no resolution.

      1.The *********** of America failed to do a final insurance pre-authorization check prior to my medical procedures on 11/7/22 and 11/9/22 and did not inform me of this.2.The *********** of America failed to provide required documentation in the claims to my insurance company that would proof my procedures on 11/7/22 and 11/9/22 were medically necessary. 3.I was recklessly misled by an estimate document given to me by the *********** of America (attached) prior to my procedures that shows 100% insurance coverage for my procedures. 4.I have all exhausted all attempts at calls, emails, and visits to the *********** of America reporting the above issues and have not had any response or action from them.5. The result of the above is that insurance claims for procedures on 11/7/22 and 11/9/22 have been denied by my insurance company and I am being held responsible for the cost.

      In several attempts, I tried to gain medical records for my records to apply for a Veteran Affairs claim for the condition that the clinic found. In this case, I have asked for records for over a month and have not received them. I have been run around each time I requested them and said I would be contacted. Even the notice of going to the local office each time they have informed me that their printer isn't connected. Each time in a 3-week period their local printers have not been connected for them to print my local records. I would like to get my records in a timely fashion as the time period for my VA claim is coming to an end to file.

      I visited I belive the name was midwest vien clinic in *********** I'll off 143 back in October 2022. I was told my consultation was free and that everything is covered by my insurance. Every time I called the number listed on the billing statement was disconnected. ************. I located there main number and have been getting the run around ever time I try to get my billing issue resolved. They keep telling me it's the balance after the insurance claim and that they will remove the 100 consultation fee. I keep trying to tell them it doesn't look like it was ever submitted to my insurance. I have spoke to perhaps 6 different people. They are rude, no helpful and have not help me out. I ask to speak to a supervisor several times either they tell me to call back or they are the supervisor. All i want to do is get this billing issue resolved. I stressed to them prior to me being seeing if it's not covered on my insurance I do not want the service. They told me several times at the clinic it was covered and I won't be charges anything. Months later I am still receiving a bill.

      I had been going to Vein Clinics of America since August of 2022, I had a few procedures, then in September 12, 2022, I went in for Sclerotherapy treatments, I paid $599.00 for a package of 4 sessions, had my first treatment that day, I have 2 more treatments left, when I called in in October to schedule my last treatment, I was informed that they were closed, and it was USA Vein Clinics, they told me to call back in November which I did then was told to call back in December, that by then the **************** would be open, not so, I called in December and was told that office would open in May! I called today and now they say they have no idea when that office will open. They never informed us what was going on, no one has reached out, they took my money and now ** left in the cold. This is unacceptable.
