This facility is understaffed, no training at all, very negligent/careless with my mother and I had her discharged 9 days after she was admitted there for rehab. So much happened with that week from giving my mom her morning meds at 2:00pm giving her dinner meds at 11:30p at night because they sent the nurse to another floor to work and she got behind, my mom would get herself to the bathroom because staff was unreliable. Not to mention a STNA took my moms oxygen off of her and put her in a wheelchair and took her to another area to get weighed. Also she woke up from a nap and her oxy was 77 because her hose was out of her machine. I had a BAD experience with the lead nurse not listening to my concerns about my mom while she was watching videos in her phone. The lead nurse gave my mom both fast & slow acting insulin without checking her blood sugars first and my moms sugars bottomed out at 9:30p that night and she broke out in a horrible sweat & called for the nurse. The ****** male ) who had been rude to her twice already walked in and said `what do you need` she told him that she needed the nurse and her sugars are to low he just replied` the nurse is doing her rounds she will be in and just left my mom lay there & did not go get the nurse. eventually they came in and gave her candy, orange juice to get her sugars up. They did not help her shower BUT put her in the shower on her 6th day there and said wash what you can and yell for me when you are done. Mind you my mom is 76 just had surgery on her knee due to being diagnosed w/septic joint. She needed help getting around & showering also therapy being there 9 days they worked with her in therapy twice. I feared that something was going to happen to her if I left her in there so I had her discharged & request PT at home thru Ohio Health. So her being there was pointless I would NEVER recommend this place to my worst enemy so please be cautious putting your loved one in this facility.Grand of Dublin Response

Thank you for your message. The standards that we expect as an organization are taken very seriously and we always want to know when someones experience was less than excellent. Our staffing ratios are excellent at ***** patients per staff member per day which is aligned and often in excess of what other centers are able to consistently staff; In addition to direct care staff, nursing managers are on duty daily to enhance care being provided and especially to be available to family members if there are questions or concerns. I am the Administrator, but also an RN, dedicated to positive clinical and service outcomes for residents and families. On 9/10/21 my wife went to The Grand for skilled nursing care and physical therapy, 10 days after having back surgery. Transfer took place on a Friday afternoon. Although aware that she was coming they did not get all of her prescription meds in until more than a day later. A physical therapist did a brief evaluation and then informed us that they did not do therapy on weekends so they would be back to work with her on Monday. Furthermore, their wound care nurse did not work on weekends to inspect and change the dressing on her surgical incision. So, for two and a half days they did nothing more than attend to her basic needs of food, changing soiled diapers, and distributing meds when they finally got them in. Nurses were understaffed. A doctor did visit in the mornings but usually before the facility was open to visitors so family members miss these visits unless you get in early. My wife was exhibiting symptoms that concerned me including confusion at night, complaining of being hot, and her legs becoming weak. I missed the doctor on Sunday morning so was not able to discuss my concerns with him. I did bring my concerns to the attention of the nurses but Im not sure what if anything was communicated with the doctor. I arrived early Monday morning but the doctor did not make rounds at all that day. Physical therapists came to do therapy and my wifes legs were so weak that she could barely stand at the bedside. (Three days earlier at the hospital she had walked about 25 feet with assistance.) My concern was growing so I decided to stay the night to make sure I did not miss the doctor in the morning. Unfortunately, my wife passed away during the night, about three and a half days after arriving at The Grand and more than 40 hours since their doctor had last seen her. My opinion is that The Grands incompetent medical care bares the responsibility for her death. I would never ever recommend this facility to anyone needing care for a loved one.Review from Linda B

My husband *** was admitted to hospital on 7/6/21 with back surgery on 7/8. During recovery he was eating, drinking, and beginning to walk with PT help and ******. He was sent to Grand at Dublin for rehab and skilled nursing on 7/15. Due to loss of appetite and drinking (quantities not documented), he soon became weak and lethargic after 2 days. Finally put on IV for fluids at my request. Due to unchanged Depends when I was not present, he developed C-diff and rashes down his thighs. On 7/23 he was asleep and coughing with flem accumulating in his mouth and I took concern to nurse who came in and partially cleared it, I finished the task. With history of pneumonia, worried about a relapse, I asked for chest xray. Did not learn until 7/26 that it showed he did have pneumonia and would start him on antibiotics. 7/28 AM I asked when he started the meds and was told it would begin that evening, 6 days after pneumonia chances/confirmed were first addressed. Then I insisted he be taken to hospital NOW. At hospital he was put on IV and antibiotics immediately. There was improvement by next day. 8/2 he had to have PeG tube inserted to give nutrition and meds, remaining in hospital for 12 days. In just 2 weeks at this facility, he went from eating, drinking, and starting to walk after major surgery at the hospital to not eating and great weight loss, dehydration needing IV's, not swallowing, completely lethargic, C-diff and pneumonia. In addition I received **** for $200 from The Grand and have recorded 12 times since 12/13 to reach billing, leaving messages without response. I want to warn others NOT TO ENTRUST LIVES OF LOVED ONES TO THE GRAND OF DUBLIN.Grand of Dublin Response
