      I was injured at work and doctor umphreys who is an amazing doctor was the only doctor to help me then my job decided to contact the director doctor hudwig I believe who changed my entire work restrictions (who has never seen or met with me ever) and has now made everything worse this has created trauma and other issues I am also going to the media and medical examiners board they only changed my work restrictions because my job asked them to they work for the job not the patient

His care and restrictions should remain in the hands of the specialists once he has been transferred to them. He has not been transferred back to our care. So Dr. Umpries was the only doctor who knew about my anxiety ptsd cuncssuoon symptoms. No doctor who has never seen me or spoke to me has the right to change work restrictions without even speaking ti me . I'm worse now then before. There were a reason those restrictions are in place. I have no head specialty and I have told them that.

      Concentra refused to provide me service for a ear ache and irritation in the back of my throat on left side. I was refused service and told those are COVID SYMPTOMS. However I went to ****************** and was told I had acute Lingual tonsil.

      Yes this is in reference to a Hearing and Vision test I took at Concentra Medical Center on 08/5/22 for my job. The thing that concerns me is how the doctor at Concentra tried to tell me my hearing in my right ear was very bad. And even on the paperwork it said both my left and right ear was bad. So I was concerned about it. So I went to another hearing doctor 18 dyas later for a second opinion. This was at Consulting Audiology Association. The doctor told me my hearing was okay. So I dont know what Concentra problem was with me. For also they had me take the hearing test twice. I believe it was a form of harrassment. I found this very disgusting. So below I have copies of the tests from both companies. And I would like to thank you for your time.

      I went to get a drug screen the workers told me the results would come in by Monday 8/8/22 and they never did then they told me to call back Wednesday morning 8/10/22 and the results weren't there. I asked for the lab information and they refused to give me the information because they were too busy. Then they asked for me to fill out paperwork after 5 days of the service date when it should have been handled the day of service date.

The patient called on Monday, ** did not have the results, and a medical release form was emailed to her. Our local operations team will be coaching the team on providing that at time of registration for private pay UDS's what expected wait time will be. They emailed the results to the patient on 8/11/2022.

      I was in an Auto Accident recently and my neck is still bothering me. I wanted to have someone take a look to see if there were any lingering issues. I called Concentra Urgent Care and they told me over the phone that they don't deal with auto insurance providers so the total would be $245 out of pocket. I thought that was weird but I needed to have it done so I accepted. They also told me it would be a 3-4 hour wait. Also not great but fine. I arrived, paid, and had my blood pressure taken. I then waited over 3 hours until my name was finally called. I was then taken back into a room where I waited for the doctor. After about 10 minutes the nurse came back and told me they don't do anything related to auto accidents, my payment had been voided and they asked me to leave. This entire ordeal took nearly 4 hours for absolutely nothing. This is ridiculous and pretty shady in my opinion.

      Went from my physical today, and they were very discriminatory toward me. After being in the office, the paperwork they gave me was to a total different patient, so I don't even know if I was the patient she was seeing she got us confused. I was sent home with his paperwork and part of mine (incomplete). How is this legal? How do I even know that she is providing the correct information? This place needs to be shut down, it really is dangerous to go somewhere where the paperwork doesn't even match the patients and I don't even know if she is reporting the test results for me or the other gentleman that I got his paperwork. If you value your health, do not go here.

      I had a visit with Concentra for a worker's compensation claim. The clinician that saw me was O. Hernandez. I suffered back to back work injuries. And did not want to keep going back to work before I was fully recovered. I tried to explain to Hernandez that minimum movement causes pain. That I was not able to do any of the so called "accommodations" she recommended. She ignored my MANY MISGIVINGS. She said not going back to work was NOT AN OPTION. It was clear that was a conclusion she reached prior to seeing me. This woman is not suitable to be a healthcare "professional" Her ONLY concern is to get people back to work at any cost. Her treatment towards me was 100% EVIL and she cared nothing about my well being

      I was sent to concentra via my employer on 6/8/22 upon my arrival I provided the paperwork needed to get my test down, there was a ignorant African American representative at the front desk ( she would've give me her name ) with individuals in who was giving me a hard time I sent the paperwork provided by my employer to concentra email I even wrote down the code for her. After filling out the paperwork I was told that it was a 2-3hr wait for what I needed down I said no problem because I need this for my job. I was waiting for a hour outside in my car when the ignorant representative called me from her personal number asking for my information again because she didn't have it I explained that I gave it to her 4 times already and proceeded to give it to her again when she yelled into the call "ugly *****" and hung up on me, me and my fiancé went inside to see what the issue was once inside she began being ignorant and getting smart so I asked for a supervisor. Well the supervisor Alex was no help abs told me that the lady just put my information in abs I would have to wait 3hrs my finance told him I've been waiting and been gave them my information he turned to my husband and said if his name isn't the name on the paperwork than he needed to be quit ! I am overwhelmed with the lack of professionalism that was displayed the ignorance is not acceptable, I will be passing this information on to a few people abs see what we can do as a community about getting this Specific building shut down

      IN DECEMBER 2021, DR. KIMBERLY ABERNETHY FRAUDULENTLY REPORTED TO SEDGWICK WORKER COMPENSATION AS WELL AS MY EMPLOYER COCA COLA THAT INJURIES WHICH OCCURED WHILE WORKING WERE NOT WORK RELATED. HOWEVER, HER CONVERSATION WITH MYSELF, THE DOCUMENTATION SHE PROVIDED TO ME, AS WELL AS HER OFFICE NOTES STATE MY INJURIES WERE INDEED WORK RELATED. AFTER BEING INFORMED OF THE FRAUDULENT FILING, I RETURNED TO HER OFFICE ON 1/18/2022 TO GET FURTHER DOCUMENTATION TO SUPPORT OUR INITIAL CONVERSATION WHICH CONCLUDED IT WAS INDEED WORK RELATED. WHILE THERE, IT WAS STRESSED TO DR. ABERNETHY SEVERAL TIMES THAT MY PERSONAL ORTHOPEDIC SPECIALIST PUT ME ON LIGHT DUTY DESK WORK DUE TO THE INJURIES TO MY ROTATOR CUFF. HOWEVER, WITHOUT GIVING AN EXAM, XRAYS OR MRIS, DR. ABERNETHY INFORMED COCA COLA I COULD LIFT 45 POUNDS (IN FAVOR OF THE COMPANY, NOT THE PATIENT) WHICH WOULD HAVE CAUSED EXTREMELY DAMAGING FURTHER INJURY TO MY SHOULDER AND BACK. AS A RESULT, COCA COLA USED THE INFORMATION DR. ABERNETHY FASLEY GAVE TO FORCE ME TO CONTINUE TO WORK WHILE INJURED. THIS CAUSED GREAT EMOTIONAL AND PHYSICAL DAMAGE WHICH LED TO A MENTAL HEALTH HOSPITAL STAY AND PT. I HAVE REACHED OUT TO CONCENTRA AND DR. ABERNETHY FOR MONTHS TO BE MADE WHOLE WITH NO RESOLTIONS. I SPOKE WITH THE COMPANY'S ATTORNEY, CHARLES O'DONNELL ON 5/18/2022. HE ASSURED ME HE WOULD GET IN CONTACT WITH ME NO LATER THAN 5/31/2022 WITH A RESOLTION BUT HAS NOT DONE SO AS OF YET. I HAVE CALLED AND EMAILED MR. O'DONELL AS WELL AS DR. ABERNETHY SINCE 6/1/2022, STILL NO RESPONSE. THIS IS ADDED TO MY ALREADY DECLINED MENTAL, PHYSICAL, EMOTIONAL AND FINANCIAL STATE. I SIMPLE WANT TO BE MADE WHOLE AND GIVEN AN EXPLANATION TO WHY SHE FALSELY REPORTED INFORMATION TO THE WORKERS COMPENSTATION COMPANY. EVEN MR. They nor their insurance company nor their insurance company's attorney, Charles O'*******, has been in contact with me since 5/18/2022. In my original complaint, I provided supported documents regarding their lack of correspondence, the fraudulent information provided by Concentra's doctor, Kimberly A********, to worker's compensation company, Sedgwick, and the true nature of my injuries. It appears Concentra knowingly provided their false response in an effort to deceive the Better Business Bureau, their current patients, as well as any potential patients into thinking they have made efforts to rectify this situation which they have not. Therefore, in any future response(s) to this complaint, I request Concentra, their insurance company, and/or their insurance company's attorney, Charles O'******* provide myself as well as the Better Business Bureau with any and all supporting documents which shows "the attorney has been in constant contact with the patient (myself)" as their response states. This request should not require a lengthy response time seeing as any proof should be at Concentra's, their insurance company's, or Charles O'*******'s fingertips as easily as my supporting documents were at mine. Although I provided supporting documents in my original complaint, I am providing them again in my response. Within these documents, I have included the one and only time attorney O'******* has contacted me via email or phone and the times I have reached out to him as well as Concentra's doctor Kimberley A******** (the doctor who provided fraudulent medical information regarding my injuries, my physical limitations, and the injuries being NON WORK RELATED even though they were to Sedgwick as well as my employer Coca Cola). Image_XXXXXXXXX (X), image_XXXXXXX (X), and image_XXXXXXX (X) support the times I have reached out to Charles O'******* as well as Dr. Kimberly A******** regarding their no communication with me since 5/18/2022. One image shows his initial and only contact with me on 5/18/2022 at 8:59am CST, another shows I reached out to via email with no response, and the last image shows I reached out to him via cellphone on 6/3/2022 at 12:09pm CST. The call log shows I left Mr. O'******* a roughly 2 minute voicemail. The call log also shows Mr. O'******* did not return my call. As you can see by the call log number sequence, I purposely grouped all calls together from each number in order to show the only call made or received from their attorney Charles O'******* during the month of June is when I contacted him on 6/3/2022. The remaining images are medical documents supporting the fact my injuries were indeed work related and I am medically and physically unable to lift 45 pounds as Dr. Kimberly A******** fraudulent informed Coca Cola without doing a complete physical exam of my injuries nor did she take xrays or MRIs. As I stated in my initial complaint, this ordeal has taken an emotional, physical, and mental toll on not only myself but my family. They are only exasperating my current mental and emotional injuries by making me relive these events. I look forward to their response. Again, I am requesting all supporting documents that prove they have been in constant contact with me as their response states.

      Unfortunately, this is not my first bad encounter with concentra. First of all for the life of me, I do not understand why an urgent care, does not provide a covid test. Covid testing should be made available, in every and all urgent care locations, specially since we are still in a pandemic, and covid kits, are not easily accessible to everyone. I arrived to the site approximately at 4:30 on 5/23/22 I was not greeted or acknowledged. Not to mention there were 3 to 4 people at the front desk. If anything they seemed annoyed. I came to get seen due a sore throat, I did an at home covid test, came back negative. Before I could finish expressing my concern, I was told we don't do covid test, thats a symptom of covid, so I had to explain I had this sore throat, for days, I ve already done a covid test, my husband just tested positive for strep, I want to also, get tested for strep. They still gave me a hard time, so then they "claimed" the system wouldn't let them finish my registration. Not to mention, their was another patient ahead of me, that was literally was just check in and seen, with no problem. So I had no other choice, but to leave and go to another urgent care. Very unprofessional, very rude. It's bad enough people are sick when they come in, it's not right, that I was treated this way, when I am coming in to get help. This staff needs a big training of professionalism, it was very clear, that they did not want to help me, and we're actually happy that I had to leave, specially being that it was very close to closing time. No one deserves to be treated this way.
